UCSF Paper Shows Big Tobacco Influenced Humanitarian Children’s Rights Agency
The tobacco industry manipulated the renowned children’s rights agency UNICEF for more than a dozen years, from 2003 until at least 2016, during which time UNICEF’s focus on children’s rights to a tobacco-free life was reduced, according to previously secret documents uncovered by UC San Francisco.
The research appears April 30, 2018, in Pediatrics, the journal of the American Academy of Pediatrics.
The humanitarian United Nations Children’s Fund (UNICEF) had formed an early collaboration with the World Health Organization to reduce tobacco use among youth and had backed the international Convention on the Rights of the Child of 1989, which established civil, political, economic, health and other protections, and has been accepted by every United Nations member except the United States. The researchers report that, as part of tobacco companies’ strategies to improve their corporate images, the industry “successfully engaged” with the children’s agency leading to less emphasis by UNICEF on tobacco control.
“UNICEF allowed itself to be manipulated by the tobacco industry,” said author Stella A. Bialous, DrPH, RN, an associate professor in the UCSF School of Nursing and longtime tobacco control expert whose research focuses on the WHO Framework Convention on Tobacco Control.
“After UNICEF loosened its guidelines on funding from the tobacco industry in 2003, it opened the door for the tobacco industry to form partnerships that appeared to undermine its involvement in tobacco control,” said Bialous, who is also with the UCSF Center for Tobacco Control Research and Education. “The tobacco industry is expert at bamboozling people and UNICEF was vulnerable to it.”
UNICEF, a 70-year-old organization established by the United Nations that is currently active in some 190 countries around the world, focuses on the rights of children worldwide.
In their research, the authors delved into documents from the Truth Tobacco Documents library, an online collection housed at UCSF of previously secret tobacco-industry documents produced mainly through litigation against tobacco companies. Additional information was garnered online, from UNICEF’s and tobacco industry websites.
According to the paper, the tobacco companies saw UNICEF as holding substantial power and influence worldwide, and stressed that it should be “monitored closely.” For example, a 1998 document noted the “potential opportunity and complication” inherent in children’s rights and discussed how tobacco control advocates could “exploit” provisions in the Convention on the Rights of the Children, adopted by the United Nations in 1989.
The paper describes a shift for UNICEF starting in the early 2000s.
Before then, UNICEF carried a strong role in tobacco control, viewing it as essential to children’s rights, and called for tobacco advertising bans, raising tobacco taxes, and health education and initiated joint youth smoking prevention projects with the WHO (World Health Organization), the researchers said. In fact, WHO and UNICEF published a joint report in 2001 on tobacco and children’s rights that emphasized the need to protect children from starting to smoke, from secondhand smoke exposure, from tobacco marketing and child labor on tobacco farms.
But after that, according to the new report, the tobacco industry began engaging with UNICEF “in an attempt to neutralize” UNICEF’s anti-tobacco advocacy. The results: UNICEF embarked on a much lower profile in tobacco control and proactively engaged in support of tobacco industry public relations initiatives. For example:
- In 2003, a tobacco-industry front group mounted a campaign purportedly to prevent child labor in the Philippines, on which UNICEF served as an advisor
- In 2010, the UNICEF office in Kazakhstan accepted money from Philip Morris International to work on child labor issues
- In 2015, UNICEF published a report titled, “Obligations and Actions on Children’s Rights and Business,” with Japan Tobacco International as a contributor; the report does not mention tobacco
The authors recommend that UNICEF restore its 2001 policy of not working with the tobacco industry or its agents and to join sister UN agencies that have already endorsed a policy not to partner with the tobacco industry.
“After the tobacco companies successfully infiltrated UNICEF, it muted the agency’s efforts to protect kids from Big Tobacco,” said senior author Stanton Glantz, PhD, UCSF professor of medicine and director of the UCSF Center for Tobacco Control Research and Education (CTCRE). He is also with the Philip R. Lee Institute for Health Policy Studies.
“It is time for UNICEF—and all UN agencies—to recognize that tobacco companies support UNICEF as part of political and public relations activities to protect its markets,” said Glantz. “It’s time for UNICEF—and all UN agencies—to adopt and enforce strict measures of not accepting funding or partnerships with tobacco.”
The first author of the paper is Yvette van der Eijk, PhD, a postdoctoral fellow in the CTCRE at UCSF. Both Bialous and Glantz are members of the UCSF Helen Diller Family Comprehensive Cancer Center.

The manufacture, use, and marketing of tobacco present a serious threat to children’s right to health. This makes the Convention on the Rights of the Child a potentially powerful tobacco-control tool and the United Nations Children’s Fund (UNICEF), which oversees the convention’s implementation, a potential leader in tobacco control. UNICEF actively supported tobacco control initiatives in the late 1990s, but since the early 2000s UNICEF’s role in tobacco control has been minimal. Using the Truth Tobacco Industry Documents library, an online collection of previously secret tobacco industry documents, we sought to uncover information on the tobacco industry’s ties with UNICEF. We found that from 1997 to 2000, when UNICEF was actively promoting tobacco control to support children’s rights, the tobacco industry saw children’s rights and UNICEF as potentially powerful threats to business that needed to be closely monitored and neutralized. The industry then positioned itself as a partner with UNICEF on youth smoking prevention initiatives as a way to avoid meaningful tobacco control measures that could save children’s lives. After UNICEF’s corporate engagement guidelines were loosened in 2003, tobacco companies successfully engaged with UNICEF directly and via front groups, including the Eliminating Child Labour in Tobacco Growing Foundation. This was part of an overall tobacco industry strategy to improve its corporate image, infiltrate the United Nations, and weaken global tobacco-control efforts. As part of its mission to protect children’s rights, UNICEF should end all partnerships with the tobacco industry and its front groups.

When I worked as a consultant
When I worked as a consultant with WHO/TFI from 1998-2004 it was bruited about that despite Unicef's signing on to tobacco control under the CRC, the agency's heart wasn't in it. Carol Bellamy was Executive Director of Unicef from 1995-2005. In her previous political career she had a small flirtation, may I say, with Philip Morris when running for the post of New York State Comptroller. PM's lobbyist in Albany met with her on October 26, 1990 to discuss her position on the state's pension fund and divestment of tobacco stocks. The lobbyist reported: "Bellamy was emphatic in her response '...I do not like cigarettes, I believe they cause health problems, my father died of cancer.'" But, she continued, her main responsibility as Comptroller would be to achieve greatest return and therefore,"I would not support disinvestment." The lobbyist went on: "She summed up her comments by saying, 'I like Philip Morris...I think it is a great company'." The lobbyist's memo continues: "Wanting to be supportive of her candidacy, I invited her to lunch at headquarters during the transition period following the election. She graciously accepted."
